On July 1, 2003 the ACGME implemented common resident duty hour requirements. The JHU Neurology Service has instituted the following policies in order to be in compliance with these requirements. Our policies are organized by rotation.

JHU Neurology Service (JHH, Sinai and JHBMC):
    1. PGY2 residents are on call no more frequently than once every four days.
    2. PGY2 residents begin daily work at 7.30 am for morning report (M-F).
    3. On non-call days interns will work no more than 12 hours.
    4. All residents will have at least one day off per week averaged over four weeks.
    5. On call days, residents admit no more than 7 new patients and 2 transfers of care. If more transfers of care occur, proportionally fewer new patients can be admitted (e.g. 4 transfers of care and 3 new patients). Patients in excess of these requirements will be admitted by the supervising senior resident.
    6. Residents will care for no more than 12 patients at any given time. If a resident is assigned more than 12 patients, the patients will be redistributed evenly throughout team. Thus the total team can care for no more than 30 patients per 4 residents.
    7. Residents will work no more than 80 hours per week.
    8. Senior residents supervise junior residents, sub-interns, medical students, and participate in patient care duties.
    9. When not on call overnight, senior residents should leave by 12 midnight when directly supervising a resident or student, or 6 pm when not in a supervisory capacity.
    10. If any housestaff is unable to complete work by 12 midnight, they should not return to the hospital for 8 hours. To facilitate this, they may miss morning rounds, and report to the hospital later.
Elective Rotations
    1. Residents' specific duty hours will vary greatly from elective to elective given the varied nature of the experiences. In general, however, residents will arrive at 8 am and leave by 7 pm Monday through Friday with no weekend duties except as delineated below.
    2. If residents are required to take call, the call period will begin at 8 am and end within 24 hours. The resident must complete work duties within 30 hours of arrival for a call period.
    3. Residents will have on average one day off per week for two week or one month electives. If the resident takes call for cross-cover or the specialty service on a Saturday, then week days must be given off on order to ensure that a total of 4 days are off averaged over 4 weeks.
    4. Residents take overnight call as cross-cover no more than every fourth night. On days after call, the resident will leave by 2 pm. In order to facilitate this, afternoon clinic responsibilities will be terminated on post call days.
    5. Total work hours must be less than 80 per week, averaged over a 4 week period.
Ambulatory Block
    1. Resident assigned to block should work no more than five days of ambulatory clinic per week.
    2. Work duties should be from 8am-6pm Monday through Friday.
    3. There will be no weekend or overnight assignment except for the Jeopardy system (five days maximum per four week period).
    4. Resident should work no more than 80 hours total per week averaged over a four week period).
    5. If resident is jeopardized for overnight call assignment then they will not work more than 30 hours contiguously and will have no afternoon clinic assignments the following day so that they will leave by 2 pm.
